Create Quizzes With Omnisend Signup Forms

Create interactive quizzes with Signup Forms, save responses to contact profiles, and use the data to personalize your marketing

Written by Chaymae

Use a Signup Form to create an interactive quiz for your website visitors. Collect preferences or needs, and save the answers on the contact profile.

You can use quiz responses to create segments, personalize messages, and trigger Automation Workflows.

Before You Begin

  • You can use any Signup Form type to create a quiz, including Popup, Flyout, Fullscreen, Embedded Form, and Landing Page.

  • Your Signup Form must include an Email or Phone Number field so Omnisend can create new contacts and save their quiz responses.

Benefits

With a quiz-based Signup Form, you can:

  • Collect contact information and preferences in one experience.

  • Save responses directly to contact profiles.

  • Build audiences based on visitor preferences.

  • Personalize Campaigns and Automation Workflows using collected data.

Create a Quiz Using Signup Forms

Step 1. Create a Signup Form

Go to Forms → Create form to create a new Signup Form. You can start with a pre-built template or generate a form with Omnisend Agent.

To use a pre-built quiz template:

  1. In the pre-built templates list,  choose a quiz template, such as Fullscreen Quiz Email & SMS.

  2. Click Edit in Form Builder to edit the template manually, or select Edit with Agent to use Omnisend Agent.

Note: Omnisend Agent for Forms is available only on paid plans.

To create a form with Omnisend Agent, describe the quiz you want to create. Include the questions, answer choices, and information you want to collect. Then, select one of the suggested form templates and review or adjust it.

💡 You can also edit an existing form, following the steps below.

Step 2. Add an Email or Phone Number field

To collect quiz responses from new visitors, add an Email or Phone Number field to your Signup Form. Omnisend uses this information to create a contact profile and associate the quiz answers with that contact.

Note: If your quiz targets existing contacts, Omnisend can recognize them and save their responses to their existing contact profiles without requiring them to enter their email address or phone number again. To use this approach, configure the form's visitor targeting to show it to existing contacts.

You can collect both email and phone number in the same quiz. If you use a multi-step Signup Form, collect the email address before the phone number or collect both fields in the same step.

💡 Remember to add Legal Consent when you need to collect GDPR or TCPA consent.

Step 3. Add Quiz Questions

Add form items based on the type of response you want to collect:

  • Input: Collect a free-text response.

  • Date: Collect a date, such as a birthday or preferred date.

  • Dropdown: Let visitors select one option from a compact list.

  • Radio Button: Display multiple options and let visitors select one.

  • Checkbox: Let visitors select multiple options.

  • Buttons: Present answers as clickable buttons.

For example, if you run a skincare brand, ask “What is your skin type?” and add Oily, Dry, and Combination as answer choices, or ask “Which products do you refill most often?” and offer options such as Daily moisturizer, Toner, SPF, etc.

You can also add Text, Image, or a Countdown timer to provide context or create urgency. If you want to make the Signup Form more interactive, add gamification items such as Wheel of Fortune, Scratch Card, or Mystery Box.

Step 4. Map Each Quiz Question to a Contact Field or Custom Property

Map each question in your form to the standard contact field or custom property to save responses to contact profiles.

Use a standard field when the answer matches an existing field, such as Country, Birthdate, First Name, or Last Name.

For information that does not match a standard field, use a custom property:

  1. Select the item you use to collect data, such as Input, Dropdown, Radio Button, or Checkbox.

  2. In the right-side menu, select an existing custom property or create a new one.

  3. Add or update the available choices, if needed.

After a contact submits the form, Omnisend saves the response to their contact profile under the selected contact field or custom property.

Important: If you use multiple buttons to collect an answer, set each button’s action to Submit form, enable Map to custom property, enter the custom property name, and assign the value that should be saved. The value is not taken automatically from the button text.

Step 5. Organize the Quiz into Multiple Steps

Use multi-steps when your quiz includes several questions. Separate the questions into focused steps so the form does not become crowded.

You can use up to four steps:

  • Set each button's action to Submit form to save the collected data on each step.

  • If your quiz targets new visitors, include an Email or Phone Number field in at least one step so Omnisend can create a contact and save quiz responses.

    • If you collect both fields, collect the email address before the phone number or collect both in the same step.

Step 6. Review Behavior and Display Settings

Review the available settings for your form type.

  • Behavior settings: Adjust when, where, to whom, and how often the form appears. This includes Display settings, such as Page visits, Time on page, Scroll depth, and Exit intent.

  • Theme settings: Adjust the form’s appearance, including its fields, buttons, layout, and styling.

Step 7. Preview and Enable Your Form

Preview the quiz on both desktop and mobile devices. Before enabling the form, check that:

  • Every question displays correctly.

  • All form items used to collect quiz answers are mapped to either a standard contact field or a custom property.

When everything is ready, click Enable form to publish it on your store.

Use Collected Data in Omnisend

Review Quiz Submissions

To review form performance, go to Reports → Forms and open the report for your Signup Form. You can review views, submissions, and conversion data.

You can also open the form’s Contact activity to review submitted contact details and saved custom properties.

Create Segments Based on Quiz Answers

Create segments based on the data saved from quiz responses.

For example, if you collect a skin type custom property, create a segment where:

  • Custom property: skin_type

  • Condition: is

  • Value: oily

You can create additional segments for the other answers. You can also combine quiz responses with standard contact fields or other contact data to create more specific audiences.

For example, target contacts whose skin_type is oily and whose Country is United States.

Personalize Campaigns and Automation

Use quiz responses to create more relevant messages across your Email, SMS, and Push Notifications.

You can:

  • Add quiz answers to messages using Personalization tags.

  • Send targeted campaigns to segments based on quiz answers. Segments can be included or excluded from Email, SMS, and Push Notification campaigns.

  • Recommend specific products based on the information collected from the quiz.

  • Use Conditional content to show different text, images, or calls to action based on contact custom properties, segments, or tags.

  • Create an Automation Workflow with the Contact enters a segment trigger to send automated messages when a contact enters a segment.

  • Use Conditional splits to send contacts through different workflow paths based on their quiz responses.

💡 For example, create a segment where skin_type is oily, then send those contacts an Email Campaign or Automation Workflow featuring products recommended for oily skin.

Quiz Use Cases

Choose a quiz format based on the information you want to collect:

  • Recommend products based on preferences: Ask about a contact’s needs, interests, or product preferences, such as skin type or the products they refill most often. Use the answers to recommend relevant products and personalize follow-up messages.

  • Collect content preferences: Ask contacts what they would like to receive, such as product updates, educational content, special offers, or messages about specific categories. Use their answers to create targeted segments and send more relevant campaigns.

  • Understand purchase intent: Learn what a contact is interested in, when they plan to buy, or what type of solution they need. Use the answers to send more relevant recommendations or follow-up messages.

  • Collect feedback: Ask for opinions, preferences, or feedback using radio buttons, dropdowns, checkboxes, or buttons.
